Ardor Health Solutions is seeking a travel Skilled Nursing Facility (SNF) Occupational Therapist for a travel job in North Hondo, Texas. This post-acute rehab opportunity offers the chance to work with adult and geriatric patients while helping them improve independence, safety, and overall functional ability.

Responsibilities

• Perform comprehensive occupational therapy evaluations and reassessments.
• Develop individualized treatment plans focused on improving ADLs and functional independence.
• Treat orthopedic, neurological, post-surgical, and medically complex patients.
• Provide ADL retraining, upper extremity rehabilitation, transfer training, cognitive interventions, and adaptive equipment education.
• Collaborate with nursing staff, physicians, PT, SLP, and case management.
• Educate patients and caregivers on safety strategies, home modifications, and discharge planning.
• Complete timely documentation and progress reporting.

Requirements

• Texas OT License Required
• BLS Required
• SNF or post-acute experience preferred
