Travel Occupational Therapist - Long Term Care - $2,126 per week

About the role

Ardor Health Solutions is seeking a travel Occupational Therapist for a 13‑week Skilled Nursing Facility (SNF) contract in Hondo, Texas. This post‑acute rehab assignment involves working with adult and geriatric patients to improve independence, safety, and overall functional ability in a collaborative rehab team.

Responsibilities

• Perform comprehensive occupational therapy evaluations and reassessments.
• Develop individualized treatment plans focused on improving ADLs and functional independence.
• Treat orthopedic, neurological, post‑surgical, and medically complex patients.
• Provide ADL retraining, upper extremity rehabilitation, transfer training, cognitive interventions, and adaptive equipment education.
• Collaborate with nursing staff, physicians, PT, SLP, and case management.
• Educate patients and caregivers on safety strategies, home modifications, and discharge planning.
• Complete timely documentation and progress reporting.

Requirements

• Texas OT license required.
• BLS certification required.
• SNF or post‑acute experience preferred.
• Ability to work full‑time (40 hours per week) in a skilled nursing facility setting.

Schedule

• Start date: 10/12/2026 (contract runs 13 weeks, ending 01/04/2027).
• Full‑time, 40 hours per week.
• Shift: 8‑hour days.
