About the Role

This position involves providing therapy and rehabilitation services at home health facilities. The role requires a certified professional with relevant certifications and skills as listed.

Responsibilities

• Assess patients' needs and develop individualized care plans
• Implement and monitor therapeutic interventions
• Document patient progress using the Oasis charting system
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Provide patient/family education and discharge planning
• Perform various procedures related to prosthetics, amputees, and neurological disorders

Requirements

• At least 2 years of experience in therapy/rehabilitation
• BLS certification

Qualifications

• Proven ability to work independently and manage multiple tasks
• Strong interpersonal and communication skills
• Ability to travel frequently within Florida
