About the Role
This position involves providing therapy and rehabilitation services at home health facilities. The role requires a certified professional with relevant certifications and skills as listed.
Responsibilities
- Assess patients' needs and develop individualized care plans
- Implement and monitor therapeutic interventions
- Document patient progress using the Oasis charting system
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Provide patient/family education and discharge planning
- Perform various procedures related to prosthetics, amputees, and neurological disorders
Requirements
- At least 2 years of experience in therapy/rehabilitation
- BLS certification
Qualifications
- Proven ability to work independently and manage multiple tasks
- Strong interpersonal and communication skills
- Ability to travel frequently within Florida
