Travel Occupational Therapist - Home Health assignment in Concord, California serving a geriatric patient population. Responsibilities

• Provide occupational therapy assessments and interventions to geriatric patients in a home health setting.
• Maintain timely communication and documentation as required.
• Meet weekly productivity point requirements.
• Adhere to the scrubs dress code (any color, no jeans).
• Use the provided iPad for work-related tasks.
• Travel within up to a 60‑mile radius of the hub office, with flexibility for assigned territory.

Requirements

• Current OT license (CA license required in hand).
• Minimum 1 year of recent home health experience.
• Current BLS certification.
• HCHB EMR experience.
• Oasis experience.
• Ability to meet weekly productivity standards.

Schedule
