Responsibilities

• Evaluate residents’ functional status and develop individualized OT treatment plans.

• Reassess residents regularly and adjust treatment plans to meet goals.
• Collaborate with an interdisciplinary team and communicate with families/caregivers.
• Support discharge planning and coordinate patient progress with ongoing therapy needs.

• Ability to provide occupational therapy services in a long‑term care setting.
