About the Role
Provide one-on-one occupational therapy in patients' homes, focusing on activities of daily living (ADLs), home safety, adaptive equipment, upper extremity function, cognitive rehabilitation, and maximizing independence. Collaborate with a multidisciplinary team including physical therapists, nurses, speech therapists, and case managers to deliver quality care and build meaningful patient relationships.
Treat diverse patient populations such as orthopedic, neurologic, post-surgical, and medically complex cases.
Responsibilities
- Deliver occupational therapy services in patients' homes.
- Develop and implement individualized treatment plans.
- Document patient progress and adjust care plans as needed.
- Contribute to interdisciplinary team efforts to improve patient outcomes.
Requirements
- Current Occupational Therapist (OT) license.
- Strong clinical skills in home health settings.
- Excellent communication, teamwork, and adaptability.
- Ability to work independently with flexible scheduling within a designated territory.
