Occupational Therapist (OT) Home Health (HH) in Fort Dodge, IA. Responsibilities
- Provide patient-centered therapy services to individuals recovering from hospitalization or nursing home stays
- Conduct patient assessments and therapy planning
- Complete documentation in accordance with organizational standards
- Travel roughly 200-250 miles weekly within the coverage area
- Participate in weekly virtual team meetings and monthly in-person meetings
Required Qualifications
- Iowa state license
- Valid driver's license
- Two years of previous Occupational Therapist experience
- Current Basic Life Support (BLS) certification
- Work history/resume required
- Skills checklist completed within the past year
- License verification required
- Copy of certifications required
- References, including one supervisory reference, required
- Must have own vehicle (mileage will be reimbursed)
- Must have a taxable address outside of 50 miles from the assigned facility
Preferred Qualifications
- Home health experience preferred but not required
- Epic (electronic medical records) experience preferred
Schedule
- 8-hour shifts, 40 hours per week
- Beginning on 08/31/2026
- No on-call requirements

- Coverage area within 50 miles of Fort Dodge, IA
- Orientation includes general organization orientation, online courses, Epic (EMR) training, and shadow visits
- Full productivity expected by week 3
- Department comprises a PTA, COTA, PT, and OTC
- Coverage includes short-term therapy services for patients post-hospitalization or nursing home stays with varied patient needs
