LanceSoft is seeking a travel Occupational Therapist for a 13-week position starting August 1, 2026 in Portland, Oregon.

Responsibilities

• Provide occupational therapy evaluations and treatment in patients' homes.
• Develop and implement individualized care plans to improve independence and daily function.
• Complete accurate OASIS assessments and documentation.
• Educate patients and caregivers on therapeutic exercises, adaptive equipment, and home safety.
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate care.
• Document all patient care using Epic Remote Client, Rover, and OASIS.
• Travel between patient homes, with routes optimized to minimize drive time (occasional travel up to approximately 60 miles per day).

Requirements

• Active Oregon Occupational Therapist License
• Minimum 1 year of skilled Home Health OT experience
• OASIS experience required
• Current BCLS certification
• First-time travelers welcome
• Must have reliable personal transportation
• Covid-19 primary vaccination series plus one booster required
