Job Details
- Schedule: 8-hour days, minimum of 40 hours per week starting on 08/03/2026
Required Qualifications
- Certification in Basic Cardiac Life Support (BCLS)
- Oregon Occupational Therapist license
- 1 year of skilled home health experience
- Valid drivers license and reliable transportation
- Covid-19 primary series vaccination plus one booster
- Oasis certification preferred
Patient Ratios and Caseload
Up to 6 patients per day with a 1:1 patient ratio, adhering to facility standards.
Additional Information
- Traveler is expected to have their own transportation
- Occasional travel radius of approximately 60 miles within the Portland metropolitan area
- The work environment involves general outpatient/home health practices, with documentation completed via EPIC charting system and Rover phone app
- Attire is business casual, with scrubs optional
- The facility supports a group of adult and geriatric patients with diagnoses including dementia, Parkinsons disease, ALS, MS, post-orthopedic post-stroke, among others
- Must be willing to float between facilities within the state as needed
- References covering 1 year of experience are required prior to start
