Job Details:
• Screen, evaluate, treat, and develop discharge plans for patients across inpatient, Long Term Care, Alzheimer's, Home Health, and outpatient settings
• Manage patient caseloads across all clinical settings from screenings and evaluations through discharges
• Collaborate with interdisciplinary treatment team to provide comprehensive patient care
• Provide necessary billing documentation for billing purposes
• Communicate effectively with physicians and other members of the rehabilitation team regarding treatment plans and patient responses
• Counsel patients and families according to degree of disability or impairment
• Complete all required documentation thoroughly and in a timely manner
• Participate in educational programs and in-service meetings
• Maintain compliance with State and Federal Regulations
• Maintain established policies and procedures, objectives, quality assurance programs, safety, environmental, and infection control standards
• Requires regular attendance and punctuality
• Quick start position
