This role is integral to supporting participants in achieving optimal independence, safety, and quality of life through therapeutic interventions that align with interdisciplinary care plans. As an Occupational Therapist, you will: Evaluate, develop, and implement personalized treatment plans to empower participants to safely remain in their homes and communities. Work in various settings, including therapy gyms, day centers, and patients' homes, which will require local travel. Conduct home-based safety assessments, provide skilled therapeutic interventions in a day center, and coordinate durable medical equipment and other supportive services. Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals to deliver comprehensive care.
