Travel Nurse RN - Case Manager, Utilization Review - $2,250 per week

The position is located in Syracuse, New York.
Responsibilities
- Facilitates patient care across the continuum of an assigned number of cases by managing, coordinating, facilitating, and monitoring resources to achieve optimal clinical outcomes, utilization of services, and financial goals.
- Oversees the discharge process for all patients within the unit/core service area.
- Integral to the team's effort to establish a safe and appropriate discharge plan/plan of care.
- Possess a working knowledge of the spectrum of hospital and post-discharge services that are available.
Requirements
- NY RN license
- BLS
- 2+ years minimum of acute care hospital experience
- 1.5+ years recent case management experience (within past 2 years) in an acute care facility of similar size and level as this facility
- Epic experience
