Position Summary
Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patients are progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process.
Position Duties
- Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.
- Makes sure patients are moving toward their desired outcomes by checking on the care they receive through assessments and/or patient records.
- Identifies and solves problems that might get in the way of good patient care.
- Is actively involved in the process of getting patients ready to leave the hospital.
Minimum Required Qualifications
- MT/compact RN license
- American Heart Association BLS
- Minimum of 2 years of nursing experience
- Utilization review experience
Prior to Experience
- Bachelor’s Degree in Nursing
- 2 years of acute care hospital experience as a case manager
