Travel Nurse RN - Case Manager

Key Responsibilities

• Provide case management services, including care planning, coordination, and discharge planning
• Assess patient needs and develop individualized care plans

Qualifications & Skills

• Active RN license and BLS certification from the American Heart Association
• Minimum of 2 years of RN hospital experience, including case management
