Travel Nurse RN - Care Manager - $3,394 per week

The Inpatient Case Manager assess, plan, implement, monitor & evaluate options & services to affect an appropriate, individualized plan for hospital patient care across the continuum of care. The Inpt CM will follow assigned patients working with team members & Social Services to prioritize & strategize each day's activities. Using independent judgment & discretion, the patient must be assessed & reassessed as an ongoing process to address the patient's broad spectrum of needs. The Inpt CM utilizes clinical knowledge & competence, positive communication skills, problem solving & conflict resolution techniques, ability to effect change, strong skills in assessment, organization & time management. The CM process requires a focus on critical thinking skills, customer service skills, setting appropriate goals & measuring outcomes to effectively ensure optimal patient outcomes with consideration to financial & health plan resources.
Requirements
- Education: Graduate of an accredited nursing school or college. BSN required
- Licensure: Current California state RN license required
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred
- Minimum of 2 years acute clinical nursing experience required
- Minimum 2 years Case Management experience is required. ED experience would be a huge asset
- CERNER AND EPIC preferred
- 5x8hr shifts weekly 8AM- 4:30PM May be a mix of weekends and weekdays based on department needs.
