Job Summary

We are seeking an experienced RN Care Coordinator/Case Manager to coordinate patient care throughout their hospital stay. The role involves collaborating with patients, families, physicians, and multidisciplinary teams to ensure quality, cost-effective care, manage discharge planning, evaluate resource utilization, and advocate for patients from admission through discharge or transfer.

Requirements

• Minimum 1 year of recent Case Management experience in an acute care setting
• Active California RN License
• CPR Certification
