Responsibilities
- Assessing patient conditions and developing personalized care plans
- Administering medications and treatments
- Educating patients and caregivers on disease management and self-care
- Monitoring patient responses and adjusting care as needed
- Coinciding with healthcare teams and documenting care activities
- Ensuring compliance with safety, infection control, and healthcare regulations
- Supporting families emotionally and providing guidance
- Maintaining effective communication with case managers and interdisciplinary teams
- Performing other duties within scope of practice
Requirements
- At least one year of recent RN experience, including six months in home health
- Proficiency in OASIS documentation
- Valid RN license and BLS certification
- Strong interpersonal, critical thinking, and technical skills
- Physical ability to move and assist patients
- Excellent communication and ability to work independently
Qualifications
- Graduation from an accredited school of nursing
- Current RN license in good standing
- BLS certification
