Travel Nurse Practitioner
Position
Monogram Health is seeking a skilled Nurse Practitioner for a travel nursing position. The ideal candidate must live within 20 miles of New Orleans or Baton Rouge.
About the role
The Monogram Travel Nurse Practitioner provides comprehensive care to underserved patients within the Monogram Health network, focusing on improving health outcomes for those with chronic illnesses like CKD and ESRD.
Responsibilities
- Conduct in-home assessments, including comprehensive annual wellness exams, focused screenings, and other visits
- Counsel and educate patients and families about benefits and programs available to help them live healthier lives
- Document items such as chief complaint, diagnosis, history, examination, medications, allergies, assessment, and plan
- Coordinate care with primary care providers, specialists, and ancillary services
- Maintain quality of care standards as defined by the practice
- Prescribe medications, order tests, and collaborate with patient’s Monogram physician
- Facilitate closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic tests
- Absorb and evaluate patient's status
- Participate in integrated care team meetings
Requirements
- Active and unrestricted RN and NP board certification
- Current and unrestricted DEA certificate, or eligibility
- Ability to travel extensively by air and utilize rental vehicles
- Proficient with medical instruments and equipment required by the work
- Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology
- Effective communication skills, both verbal and written
- Sound understanding of all federal and state regulations including HIPAA and OSHA

Qualifications
- At least 2 years of Nurse Practitioner experience
Skills
Knowledge of disease diagnosis and prevention, ability to assess patient's health status, implement a plan consistent with appropriate plan of care, follow-up and evaluate patient's status, and participate in integrated care team meetings.

This is a travel position with up to 75% travel statewide and/or nationwide.
