Requirements
- Minimum of 2+ years of clinical experience in home health nursing
Responsibilities
- Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams
- Communicate updates to providers to facilitate clinical decision-making
- Document patient care, responses, and communications accurately in medical records
- Administer therapies, monitor for adverse reactions, and observe changes in patient conditions
