Travel assignment for a Medical Social Worker (MSW) in Healdsburg, California, focusing on acute-care inpatient settings.

Position Overview

This opportunity is ideal for a compassionate, resourceful social worker passionate about patient advocacy, discharge planning, care coordination, and connecting patients with essential community resources. You’ll work closely with patients, families, physicians, nurses, case management teams, and community organizations to ensure safe transitions of care and appropriate access to services. This assignment provides an excellent opportunity to expand your experience across acute care, rehabilitation, LTAC, discharge planning, and case management.

Responsibilities

  • Complete comprehensive biopsychosocial assessments
  • Identify psychosocial barriers affecting patient care and discharge
  • Develop individualized plans to address social, emotional, financial, and environmental needs
  • Coordinate appropriate services and resources for patients following discharge
  • Assist patients and families with discharge planning
  • Coordinate home health services, durable medical equipment, and other post-acute resources
  • Facilitate referrals to rehabilitation facilities and long-term care facilities
  • Connect patients with appropriate community programs and social services
  • Participate in multidisciplinary care conferences
  • Document pertinent patient information to support care coordination
  • Collaborate with physicians, nurses, case managers, therapists, and other healthcare professionals
  • Provide patient and family education regarding available resources and services
  • Advocate for patients and assist them in navigating complex healthcare and social service systems
  • Refer patients to appropriate internal and external disciplines and community resources

Care Coordination & Discharge Planning

A major component of this position is helping patients successfully transition from the hospital to the appropriate next level of care. Responsibilities include:

  • Home care coordination
  • Durable medical equipment coordination
  • Rehabilitation referrals
  • Long-term care referrals
  • Nursing home placement
  • Community resource referrals
  • Post-acute service coordination
  • Discharge planning
  • Care conference participation
  • Documentation supporting transitions of care

  • Coordination of post-acute services
  • Documentation of psychosocial needs
  • Communication with interdisciplinary care teams
  • Resource identification and referrals

Qualifications

  • Master of Social Work (MSW) required
  • Minimum 1 year of relevant social work experience
  • Experience in a healthcare setting preferred
  • Medical social work experience preferred
  • Acute-care/inpatient experience is highly desirable
  • Strong assessment and care coordination skills
  • Excellent communication and interpersonal skills
  • Strong documentation and organizational abilities
  • Ability to advocate effectively for patients and families
  • Comfortable working with multidisciplinary healthcare teams

Certifications

  • MSW required
  • BLS required

Schedule

  • Day Shift
  • 5 × 8-hour shifts (8:00 AM – 4:30 PM)
  • No block scheduling
  • 13-week assignment
