About the Role
We are seeking an LPN / LVN for a Home Health role, providing skilled nursing services under RN supervision. This position prioritizes compassionate care and supports nurses with expert guidance and 24/7 assistance to ensure success in home health settings.
Responsibilities
- Monitor patient status and report changes promptly
- Document care accurately
- Deliver skilled nursing care in accordance with patient needs
- Assess patient needs and ensure compliance with policies and regulations
- Perform wound care, injections, medication administration, and vitals monitoring
Requirements
- Active, unencumbered LPN/LVN license
- Current BLS certification
- At least 1 year of recent experience in home health or related fields
- Proficiency in wound care, injections, medication administration, and vitals
- Familiarity with OASIS-E domains
- Reliable transportation
