Responsibilities
- Provide discharge planning, psychosocial interventions, assessment, advocacy, referral, and interdisciplinary collaboration for patients.
- Prepare social histories.
- Assess patient needs.
- Manage policies for social, psychological, and emotional goals.
- Document services in compliance with regulations and payer guidelines.
Qualifications
- Minimum 2 years of experience
- Independent decision-making skills
- Excellent communication with staff, physicians, patients, and families
- Master’s degree from a CSWE-accredited school preferred
- LCSW credential required for most travel/contract positions
