About the role
We are seeking a qualified Licensed Clinical Social Worker (LCSW) for a discharge planning role in inpatient acute care.
Responsibilities
- Manage a caseload of 15-20 patients daily
- Collaborate with a team to ensure effective discharge processes
- Utilize Epic EMR for charting
Requirements
- At least 2 years of acute inpatient case management experience, including 1 year in a large hospital (500+ beds)
- Recent experience within the last two years
- Strong computer skills
- Epic EMR proficiency
- Experience in ED discharge planning
- Quick adaptability
- Minimal training required
- May include weekend and holiday work
Qualifications
- BLS certification
- Fingerprinting and vaccination compliance necessary
