• Registered Nurse, LD
• Licenses: RN (Compact Accepted)
• Certifications: ACLS-AHA (Copy Needed); BLS-AHA (Copy Needed); NRP (Copy Needed); CERT-FHM (Copy Needed); CERT-STABLE (Copy Needed)
• SSN Required
• DOB Required
• References: 1 Reference in entire work history
• AM will send required Sagewest form to be signed and returned from the HP in order for file to be sent
