Requirements

• 2+ years current Physical Therapist Inpatient experience (Required)
• At least 6 months previous travel experience (Highly Preferred)
• Cerner experience (Preferred)
• Traveler must have their own vehicle for this need
• Traveler’s permanent address must be included in profile
• Any/all requested time off must be included at time of submission; facility will not approve scheduling requests after offer or interview

• Work History
• Skills Checklist/Leadership Physical Therapy, current to 1 year (Required document)
• 1 Supervisory Signed Reference from within the past two years of employment (Required document)
• Active American Heart Association BLS (Required document)
• Active Vermont Physical Therapy license (Required document)

Responsibilities

• Part of shift/holiday rotation required
• No on-call duty
• Note: Mt Ascutney 3-strike rule for call-offs: after 3 call-offs, traveler’s contract may be terminated at the facility’s discretion

Mt. Ascutney Hospital and Health Center is a 35-bed critical access community hospital located in Windsor, Vermont. It has been named winner of the Foster G. McGaw Prize for Excellence in Community Service and is one of only two CARF-accredited facilities in Vermont.

The hospital features 25 inpatient medical-surgical beds, a 10-bed inpatient rehabilitation unit, a therapeutic pool, physical therapy department, acute care, transitional care, and hospice services.
