Travel Home Health Physical Therapy Assistant - $1,953 per week

VyTalent Solutions is seeking a travel Home Health Physical Therapy Assistant for a contract position in Greenwood Village, Colorado.

About the Role

This is a 13-week home health contract with a schedule of Monday through Friday, 8:00 AM to 5:00 PM. The role offers a pay range of $1,600–$1,700 per week for 40 hours of work. Both travel and local candidates are accepted.

Responsibilities

• Provide one-on-one physical therapy services to patients in their homes.
• Complete patient assessments and document progress toward therapy goals.
• Develop and adjust individualized care plans based on patient needs and response to treatment.
• Coordinate care with families and other healthcare team members to support patient outcomes.
• Educate patients and caregivers on home exercise programs and safe mobility strategies.

Requirements

• 1 year of Medicare-certified Home Health experience.
• 1 year of HCHB EMR experience.
