American Traveler is seeking a Home Health Physical Therapist to support an EMR transition and provide preceptor support to staff and travelers during system go-live.

Assignment Overview

• Setting: Home health
• Shift: Days, 5x8hrs, Monday through Friday, 8:00 AM to 4:30 PM
• Hours: 40 hrs/wk
• Start Date: October 12, 2026
• Duration: 6 weeks
• Openings: 1
• Role primarily focused on precepting and providing real-time guidance during EMR go-live rather than full-time bedside care
• Potential ride-along support during implementation phase
• Three weeks of HCHB PointCare training followed by three weeks of on-site implementation and go-live support
• Training includes in-person sessions Tuesday through Thursday and virtual sessions on Monday and Friday

Requirements

• Minimum one year of home health experience required
• Oregon license required if applicable; license pending is acceptable
• Must be tech savvy, reliable, personable, and comfortable supporting and training others
• No time off accepted during this short contract
• First-time travelers may be considered depending on experience level

Preferred Qualifications

• HCHB PointCare or mobile app experience preferred
