Travel Home Health Physical Therapist - $2,926 per week

Ardor Health Solutions is seeking a travel Home Health Physical Therapist for a 13-week contract in Silverdale, Washington. This full-time, field-based role provides one-on-one care to adult and geriatric patients in their homes, offering autonomy and the flexibility of home health practice.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on physician orders and patient goals.
• Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
• Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
• Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
• Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
• Complete timely documentation and maintain home health compliance standards.

Requirements

• Washington Physical Therapist license (required)
• BLS certification (required)
• Home Health experience (preferred)
• OASIS and HCHB experience (preferred)

Schedule

13-week contract, full-time (40 hours per week), 8-hour day shifts. Start date: 6/24/2026; end date: 9/29/2026.
