Responsibilities

• Provide skilled physical therapy services to patients in a home health setting.
• Develop and implement personalized care plans based on patient needs.
• Collaborate with healthcare team members to optimize patient outcomes.
• Document patient progress and adjust treatment plans accordingly.
• Educate patients and caregivers on therapeutic exercises and procedures.

Requirements

1 year of Medicare certified Home Health experience and 1 year of HomeCare HomeBase (HCHB) EMR experience.

Pay

Pay range: $2,720.80 – $2,856.84 (based on 8‑hour shifts and 40 hours per week). Pay package includes a tax‑free stipend amount to be determined.

Schedule

Shift: 08:00 – 17:00 (8 hours). 40 hours per week. Contract length: 12–13 weeks (as noted in the posting).
