Responsibilities

• Provide outpatient physical therapy services to home health patients
• Conduct assessments and develop individualized care plans
• Document patient progress and adjust treatments as needed
• Collaborate with healthcare team members to optimize patient outcomes

Requirements

• 1 year of Medicare Certified Home Health experience
• HCHB EMR experience highly preferred

Pay & Schedule

• Pay Range: $2,500–$2,700
• Schedule: 8:00 AM to 5:00 PM, 40 hours per week
• Duration: 13 weeks
• Start Date: 09/14/2026
