About the role
LanceSoft is seeking a travel Physical Therapist for a position in Medford, Oregon.
Responsibilities
- Provide physical therapy services to patients in their homes.
- Adhere to OASIS guidelines and other relevant standards.
- Document patient progress and treatment outcomes using Epic/OASIS.
- Ensure all treatments are performed safely and effectively.
- Communicate regularly with referring physicians and other healthcare providers.
Requirements
- Minimum 1 year of Skilled Home Health PT experience.
- OASIS experience required.
- BLS certification required.
- Active PT license (pending acceptance).
- Must have reliable transportation.
- Epic/OASIS experience preferred.
- Covid primary series + one booster required.
- First-time travelers not accepted.
Qualifications
- Current active PT license.
Skills
- Skilled in providing physical therapy services in home health settings.
- Familiarity with OASIS guidelines and other relevant standards.
- Experience with documentation using Epic/OASIS.
- Strong communication skills.
