Travel Home Health Physical Therapist - $2,754 per week

Wellspring Nurse Source is seeking a travel Home Health Physical Therapist to provide care primarily in inpatient and home health settings in Cody, Wyoming.

Responsibilities

• Deliver physical therapy services to patients in inpatient and home health environments.
• Travel to patient homes using a provided vehicle.
• Maintain accurate documentation in electronic medical records.

Qualifications

• Minimum of 2 years experience as a Physical Therapist.
• Current BLS/BCLS certification required.
• Experience with EMR systems (preferred: EPIC, Net Health, PCC, Maestro).

Schedule

• Start date: October 12, 2026.
• 13‑week assignment.
• 40 hours per week, 8‑hour days.
• Days: Monday to Friday, with rotating Saturday coverage and required holiday coverage.

Compensation & Benefits

• Pay based on 8‑hour shifts and 40‑hour weeks (tax‑free stipend amount to be determined).
• Vehicle provided for patient visits.
• Mileage reimbursement at a standard rate.
