About the role
CoreMedical Group is seeking a travel Home Health Physical Therapist for a 13-week assignment starting immediately in San Luis Obispo, California.
Responsibilities
- Provide physical therapy services to patients in their homes
- Develop and implement individualized treatment plans
- Evaluate patient progress and adjust treatment plans as necessary
- Document patient care and treatment sessions accurately
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
Requirements
- Current state licensure as a Physical Therapist
- Bachelor's degree in Physical Therapy
- At least 1 year of relevant clinical experience
- Passing scores on the National Physical Therapy Examination (NPTE)
Qualifications
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Valid driver’s license and reliable transportation
Skills
- Proficient in using electronic medical records (EMR) systems
- Knowledgeable in home health care protocols and standards
- Ability to adapt to various patient needs and environments

Pay
