About the Role

Pride Health is hiring a Physical Therapist (PT) to join a growing Home Health team. This is a 14-week travel assignment starting 08/24/2026, with a schedule of 40 hours per week, 8-hour day shifts (Monday–Friday, 8:00 AM–4:30 PM). Mileage between patient visits will be reimbursed.

Responsibilities

• Evaluate patients and develop a Plan of Care to improve physical functioning, helping patients meet or exceed goals to remain safe at home.
• Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments.
• Develop individualized treatment plans, improve patients' functional mobility, and meet a productivity expectation of 30 points.
• Deliver evidence-based therapy interventions and accurately monitor and document patient progress in Homecare Homebase (HCHB)/PointCare.
• Collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards.

Requirements

• Home Health Physical Therapist experience.
• Active PT certification.

Preferred Skills

• Experience with HomeCare Homebase (HCHB).

Pay

Up to $2,700/week gross (includes weekly non-taxable stipend).
