About the role
Pioneer Healthcare Services is seeking a travel Home Health Physical Therapist for a 39-week assignment in San Diego, California. This on-site position offers day shifts of up to 40 hours per week in a home health setting.
Responsibilities
- Assess patients' physical abilities and develop personalized treatment plans in the home setting
- Provide therapy to improve mobility, strength, and functional independence
- Collaborate with patients, families, and caregivers to support rehabilitation goals
- Educate patients and families on exercise programs and safety measures
- Monitor and document patient progress and adjust treatment plans as needed
- Implement interventions tailored to the home environment and lifestyle
- Coordinate care with healthcare teams and community resources for comprehensive support
Requirements
- Doctorate in Physical Therapy
- Active California Physical Therapist (PT) license
- Current BLS/CPR Certification
- Clean driving record
Pay
$2,657.66 - $2,739.86 per week
Schedule
- 8-hour day shifts
- 40 hours per week
- 39-week assignment starting 08/31/2026
