Travel Home Health Physical Therapist - $2,680 per week

About the role

American Traveler is seeking a Home Health Physical Therapist for a 13-week travel contract in Portland, Oregon.

Responsibilities

• Serve West Portland, OR and surrounding areas
• Provide care to patients with 1:1 ratio and up to 6 patient visits per day
• Work 5x8-hour day shifts (8:00–16:30)
• Drive up to a maximum radius of roughly 60 miles daily
• Perform charting in Epic (remote client on laptop) and Rover (mobile app)
• May be required to float between facilities within the state

Requirements

• Minimum 1 year of skilled home health PT experience
• Current BLS certification
• Epic EMR experience
• Active PT license required; pending license accepted
• At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting
• Full SSN and DOB required
• Primary COVID-19 vaccination series plus one booster required
• Licenses required if applicable
