Travel Home Health Physical Therapist - $2,664 per week

MedPro Healthcare Allied Staffing is seeking a travel Home Health Physical Therapist for an assignment in Portland, Oregon.

Responsibilities

• Assess referred patients for rehabilitation services, including assessing medical condition, functional capabilities, limitations, and restrictions.
• Administer a treatment program with specific goals determined by the patient’s capacity and tolerance under the direction of the Physician.
• Educate patients and families on appropriate physical therapy methods.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Achieve and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Requirements

• Hold a Doctorate degree in Physical Therapy OR a Bachelor's degree in Physical Therapy and have passed the NPTE.
• Graduated from an accredited school.
• Minimum one year's experience.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Valid drivers license and reliable transportation.
• Other requirements to be determined by our client facility.

Pay

Based on 12 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
