Travel Home Health Physical Therapist - $2,656 per week

LanceSoft is seeking a travel Home Health Physical Therapist for a 20-week position starting August 24, 2026, in Dunedin, Florida.

Responsibilities

• Assessment of patients' physical conditions
• Cardiopulmonary/Circulatory, Ortho, Neuro, and Home Exercise Program management
• Documentation of patient progress and treatment plans
• Equipment and Devices (crutches, gait belts, hospital beds, rolling walkers, should slings, sliding boards, soft/hard collars, standard walkers, supplemental oxygen delivery) usage and maintenance
• Medications management and patient/family education
• Signs & Symptoms physical assessment, including safety and injury prevention
• Evaluation and reporting of signs and symptoms related to physical, sexual, and psychological abuse
• Home skilled visits - 6-8 per day (minimum of 6 points per day, may increase by 1 point every 3rd weekend)
• Self-scheduling of visits and hours based on the needs of the HomeCare office

Requirements

• 2 years of home health experience
• OASIS training preferred
• BLS certification
• State license
• Valid drivers license and car for home visits
• Certified Physical Therapist license
• Credentials: BLS, state license
• Scrubs: Any solid color
• Required equipment: Laptop, MiFi for internet access, portable printers, BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system
• COVID vaccination or exemption form on file
• Mandatory mask wearing at all times while at work

Qualifications

Must have valid drivers license and car to make home visits. Must be COVID vaccinated or with exemption form on file. Must have flu shot or declination and in both cases mandatory mask wearing at all times while at work.
