Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments in patients' homes.
- Develop individualized treatment plans focused on improving mobility, strength, balance, and independence.
- Treat patients with orthopedic, neurological, post‑surgical, cardiopulmonary, and medically complex conditions.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Complete timely documentation while maintaining quality standards.
Requirements
- Arizona PT license (required)
- BLS certification (required)
- Home health experience (preferred)
- OASIS experience (preferred)
Schedule
Start Date: 10/12/2026. Duration: 13 weeks. Full‑time, 40 hours per week, 8‑hour day shifts.
