Care Team Solutions is seeking a travel Home Health Physical Therapist for a 13-week assignment starting August 23, 2026, in Newton, Massachusetts.
Responsibilities
- Provide physical therapy services to patients in their homes.
- Develop and implement individualized treatment plans based on patient needs.
- Assess patient progress and adjust treatment plans accordingly.
- Document all patient interactions and treatments accurately.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
Requirements
- Must have at least 2+ years of recent experience in Home Health Physical Therapy.
- Current licensure to practice as a Physical Therapist in Massachusetts.
- Pass required background checks and drug screenings.
Qualifications
- Master’s degree in Physical Therapy from an accredited program.
- Current certification in CPR and First Aid.
- Experience with home health equipment and techniques.
- Excellent communication and interpersonal skills.
Skills
- Strong clinical judgment and problem-solving abilities.
- Effective time management and organization skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office and electronic medical records systems.
