Travel Home Health Physical Therapist - $2,606 per week
About the role
This position provides 1:1 physical therapy in patients’ homes, giving the therapist greater autonomy and the opportunity to provide individualized care without the pace and competing demands of a traditional facility setting. The PT will evaluate patients in their home environment, develop individualized plans of care and help patients improve mobility, strength, balance and safety while working toward greater independence.
Lewistown is located in Central Montana, surrounded by several mountain ranges and wide-open Montana scenery. It’s a particularly good assignment for someone who enjoys hiking, fishing, camping, wildlife and outdoor recreation and would rather spend their days off in the mountains than sitting in traffic questioning every life decision that led them there. The smaller community also pairs nicely with Home Health for therapists who enjoy independence, driving between patients and getting outside of a traditional clinic or facility environment.
Responsibilities
- Home Health PT evaluations and reassessments
- Development and progression of individualized plans of care
- Therapeutic exercise and strengthening
- Gait and balance training
- Transfer and functional mobility training
- Fall-risk assessment and prevention
- Home safety assessment
- Assistive device recommendations and training
- Patient and caregiver education
- Coordination with nursing, OT, SLP and other members of the Home Health team
- Documentation and communication of patient progress
