About the role
We are seeking a Home Health Physical Therapist to provide high-quality care to patients in their homes. The therapist will be responsible for evaluating and implementing personalized treatment plans to facilitate patient recovery and rehabilitation.
Responsibilities
- Conduct thorough evaluations of patient capabilities and needs.
- Develop and implement individualized treatment plans.
- Monitor patient progress and adjust treatment as necessary.
- Provide education and support to patients and their families.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Requirements
- Minimum of 1 year of skilled home health experience required.
- Certification in Basic Cardiac Life Support (BCLS) required.
- New graduates are welcome to apply.
- Must have reliable transportation for patient visits.
- Primary series COVID vaccination plus one booster required.

Pay
Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
Schedule
Schedule Day Shift: 5 days per week from 08:00 AM to 04:30 PM (8-hour shifts).
