About the role
SkyBridge Healthcare Therapy is seeking a Travel Home Health Physical Therapist for a 13-week contract in Hazle Township, Pennsylvania.
Responsibilities
- Provide physical therapy services to patients in their homes
- Develop and implement individualized treatment plans
- Evaluate patient progress and adjust treatment plans accordingly
- Document patient care in accordance with regulatory requirements
- Collaborate with interdisciplinary teams to coordinate patient care
Requirements
- 1 year full-time PT Home Health experience within the last 2 years
Qualifications
- Current licensure to practice as a Physical Therapist in the state of Pennsylvania
- Passing score on the National Physical Therapy Examination (NPTE)

Pay
- Pay rate range: $49.50 - $58.50 per hour
- Pay rate is dependent on seniority and other factors that will be discussed during the hiring process
- Pay package includes a tax-free stipend
Schedule
- 40 hours per week
- 8-hour shifts
