Travel Home Health Physical Therapist - $2,406 per week
Ardor Health Solutions is seeking a Physical Therapist for a 13-week travel assignment in Lake Zurich, Illinois, covering the northern and northwestern Chicago suburbs. This role provides one-on-one physical therapy to patients in their homes on a Monday–Friday, 8 AM–5 PM schedule with no on-call requirements.

This is a larger Home Health territory covering portions of the northern and northwestern Chicago suburbs, extending from areas such as Evanston and Highland Park west toward Marengo. Daily drive times may be significant depending on census and patient locations. Candidates should be comfortable managing a larger geographic territory.
Responsibilities
- Complete PT evaluations and reassessments in patients' homes
- Develop individualized plans of care
- Provide gait, balance, strengthening, transfer, and mobility training
- Treat orthopedic, neurological, post-surgical, and medically complex patients
- Provide fall-prevention and home-safety education
- Educate patients and caregivers
- Collaborate with the interdisciplinary Home Health team
- Complete timely clinical documentation
Requirements
- Graduate of an accredited Physical Therapy program
- Current Illinois PT license or ability to obtain prior to start
- Current BLS certification
- Previous Home Health experience preferred
- OASIS experience preferred
- Valid driver's license and reliable transportation
- Strong evaluation, documentation, and time-management skills
- Comfortable independently managing a Home Health territory
