Supplemental Health Care is hiring a Home Health Physical Therapist for a 13-week contract starting ASAP in Paris, Texas. The position requires an active, unrestricted Physical Therapist license, current BLS certification, 1+ year of recent home health experience, and familiarity with Medicare-certified home health guidelines and OASIS-E documentation.
- Active, unrestricted Physical Therapist (PT) license in the state of assignment
- Current BLS certification (AHA)
- 1+ year of recent home health experience (within the last 2 years)
- Familiarity with Medicare-certified home health guidelines and OASIS-E documentation
- Experience conducting PT evaluations and developing therapy plans
- Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
- Cleared background check, driving record, and up-to-date health clearance (TB, immunizations)
- Reliable transportation with valid auto insurance and ability to travel between patient homes

Qualifications

Key Responsibilities
As a Home Health Physical Therapist, you will:
- Perform initial and ongoing PT assessments and develop individualized plans of care
- Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
- Conduct gait and balance evaluations, home safety assessments, and mobility training
- Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
- Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care
- Document all care in compliance with Medicare, OASIS-E, and agency standards
