Job Description

Ardor Health Solutions is seeking a Home Health Physical Therapist for a 13-week travel contract in Medford, New York, serving patients throughout the Central and South Suffolk County/Patchogue area of Long Island.

Requirements

• Graduate of an accredited Physical Therapy program
• Current New York PT license
• Current BLS certification
• Previous Home Health experience required
• HCHB experience preferred
• Comfortable independently managing a Home Health caseload
• Comfortable traveling throughout Central/South Suffolk County
• Strong documentation and time-management skills

• Homecare Homebase (HCHB) documentation
• Up to approximately 250 miles of travel per week
