Travel Home Health Physical Therapist - $2,150 per week

Job Summary

The position of Physical Therapist Home Health is located in Lexington, Kentucky. This role involves providing physical therapy services within a home health setting under a post-acute care model. Job Details

• Shift: Dayshift, 8-hour shifts, Monday through Friday, with rotating weekends.
• Minimum Hours Per Week: 40 hours
• Start Date: 07/27/2026

Required Qualifications

• Licensure to practice in the state of Kentucky
• BLS (AHA) certification
• 1 year of experience minimum

Preferred Qualifications

• Home care home health experience
• Experience with home base settings
• Experience with common diagnoses such as CHF, orthopedic conditions, and pneumonia
• No new graduates preferred

Additional Information

• No clinical involvement
• Minimal orientation with two weeks of preceptorship
• Phone and tablet provided
• Minimum of 6.5 patient visits per day including start of care, recertifications, evaluations, and daily visits
• Uniform either scrubs (color not specified) or business casual
• Resource support available 24/7 from clinical management or educators
• Holiday rotation and weekend work required
• Charting system used is Homecare Homebase (HCHB)
• The hospital can cancel up to 4 shifts per 13-week assignment or 3 shifts for shorter assignments

Pay Package
