About the role

We are seeking a compassionate and experienced Occupational Therapist (OT) to provide high-quality home health services to patients in Salisbury and surrounding coverage areas. The OT will conduct in-home evaluations, develop individualized treatment plans, provide therapeutic interventions, perform reassessments, and complete discharge planning to help patients achieve the highest possible level of independence and functional ability.

Responsibilities

• Conduct in-home OT evaluations, treatments, reassessments, and discharge visits to help patients achieve optimal functional independence and outcomes.
• Develop and implement individualized treatment plans, including ADL training, therapeutic interventions, safety assessments, and recommendations for adaptive equipment or home modifications.
• Complete accurate and timely clinical documentation while maintaining compliance with home health regulations and agency policies.
• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated, patient-centered care across the assigned coverage areas.

Requirements

• Active Maryland Occupational Therapist (OT) License and required BLS/CPR/ACLS certification.
• Strong clinical skills with the ability to independently manage patient care in a home health setting.

Qualifications

Preferred Qualifications:

• Previous Home Health experience, including OASIS documentation.
• Experience with Homecare Homebase (HCHB) and PointCare for clinical documentation.

Schedule

• Start Date: 09/25/2026
• Duration: 14 weeks
• 40 hours per week, Monday–Friday, 8:00 AM–4:30 PM
• On Call/Weekend Requirement: one weekend a month if possible.
• Mileage Reimbursement: Yes, between patient visits (from first patient to last patient)

Pay

Traveller Pay package (Applicable only if your home location is more than 50 miles from Salisbury, MD, 21801):

• Gross Pay: $61.15/Hour (Weekly: $2,446.00)
• Take Home pay: $53.95/Hour (Weekly: $2,158.00)
• Pay Structure:
  • Stipend: $1246.00/Week ($31.15/Hour)
  • W-2 Rate: $1200.00/Week ($30.00/Hour)

Local Pay (if your home location is less than 50 miles from Salisbury, MD, 21801): $50.00 to $55.00/hour (W-2)
