Job Details

13 week assignment at Wentworth-Douglass Hospital's main campus in Dover, NH.

• Shifts: 4x9 shifts from 7a-4:30p.
• Weekend Shifts: 1-2 required over the length of the contract.
• Weekday Call Shifts: 3-5 required over the length of the contract.

Job Requirements

• Primary Source Verification of Professional License/Certification: Within 30 days of start date and updated upon expiration.
• N95 Fit Test: Within 365 days to start date and updated annually; no fit testing can be done onsite.
• Respiratory Medical Clearance Attestation: Required if fit test is required for skill.
• Documentation of MMR Vaccination or Proof of Immunity.
• Documentation of TB Skin Test Screening: Within 3 months of start date or negative QuantiFERON TB Test/T-spot within 3 months of start date, updated if >6 month break.
• Physician Health Statement: Within 365 days of start date and updated if >6 month break.
• MGB Criminal Background Check: Within 45 days prior to start date and updated if >6 month break.
• MGB Resume: Including past 7 years of work experience with city & state.
• MGB Skills Checklist: Completed within 1 year of submission.
• MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
• MGB Education Verification: Through background check company.
• MGB SSN Trace: Run with Criminal check.
• MGB National Sex Offender Check: Ran within 30 days of start and updated annually.
• MGB Exclusion Checks: Ran within 30 days of start and annually.
• MGB New Hampshire State Checks: Ran within 30 days prior to start date and updated if >30 day break.
• MGB Beeline Attestation: Completed by all new applicants.
• Schedule: 4x9 shifts from 7:00 AM to 4:30 PM. 1-2 weekend shifts required over the length of the contract. 3-5 weekday call shifts required over the length of the contract.

Additional Details

• MA Conditional Licenses not accepted.
• BON required for nursing licenses.
• No religious or medical exemptions allowed for N95 Fit Test.
• Fit Test updates can be done at Martha's Vineyard & Nantucket if traveler extends beyond original assignment due to limited resources.
• County Resided: Include any counties resided, employed, and attended school within the last 7 years for background check.
• International Residence or Travel: Must be included in background check if exceeding 30 consecutive days within last 7 years.
• Potential Matches in Background Checks: Must be vetted with MGH/Security via secure/encrypted email.
• Government Websites Used for Exclusion Checks: MassHealth, NH Medicaid, OIG, and GSA.
• Two Separate NH Checks Required: NH Criminal Background Check and NH BAAS Check.
• Name Change Documentation Required: If credential documentation does not match current legal name or profile.
