Job Requirements

• Primary Source Verification of professional license and/or certification required within 30 days of start date; MA Conditional Licenses not accepted.
• TB skin test screening within 3 months of start date and updated if >6 month break, or negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break.
• Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break.
• Fit Test for N95 respirator within 365 days of start date and updated annually; no exemptions allowed.
• Respirator Medical Clearance Attestation required if fit test is required.
• MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break.
• MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break.
• MGB Resume including past 7 years of work experience required for submission.
• Two references within 4 years of submission, minimum one supervisor or above in skillset.
• MGB Skills Checklist must be completed within 1 year of submission.
• MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions upon acceptance of assignment.
• MGB Education Verification through background check company required.
• MGB SSN Trace must be run with criminal check.
• MGB National Sex Offender Check run within 30 days of start and updated annually.
• MGB Exclusion Checks run within 30 days of start and annually.
• Documentation of MMR vaccination or immunity required.
• Hepatitis B vaccination strongly preferred but optional.
• Tdap vaccine within 10 years preferred but optional.

Additional Details

• No religious or medical exemptions allowed for N95 fit testing.
• Fit test updates can be done at Martha's Vineyard & Nantucket if traveler extends beyond original assignment due to limited resources on the islands.
• Positive PPD requires negative chest x-ray documentation.

Schedule Information
